MobilityThumbrotationoppositionLow riskPhases 1, 2, 3, 4

Goal

Encourages multi-planar thumb motion without sustained end-range loading.

Motion taxonomy (reference)

Also called: thumb big circle · thumb circles · thumb roll

Structures:

Muscles — adductor pollicis, opponens pollicis, thenar muscles, thumb extensors, thumb flexors

Tendons — thumb opposition tendon system, thumb tendon system

Indexed benefits: helps joint mobility · improves thumb mobility · improves thumb rotation control · reduces stiffness · supports fine motor movement · supports joint glide

Common contexts: arthritis stiffness · fine motor rehab · stiffness · thumb dexterity · thumb ROM · thumb stiffness · warm-up

Best for

  • Warm-up
  • Coaxing smooth thumb paths

Default dose

Reps5
Frequency3× daily
Sets / time

5 reps • 3×/day

Avoid when

  • Acute CMC flare

Measurement targets

  • Circle diameter (comfort)

Setup

  • Elbow bent; thumb tip hovering near the index MCP.

Steps

  1. 1Trace a small circle with the thumb tip.
  2. 2Reverse direction each round.
  3. 3Keep the circle smooth and small.

Cues

  • Shoulder relaxed.
  • No pinch pain.

Common mistakes

  • Making the circle so large it irritates the CMC.

Stop if you feel

Stop rules

  • Sharp pain (≥ 4/10)
  • Increasing swelling during or after
  • New or worsening numbness or tingling
  • Color change in fingers (pale, blue, red)
  • Wound opens, drains, or feels hot
  • Next morning is worse than the day before

Progressions

  • Slightly larger circle if tolerated.

Regressions

  • Support thumb metacarpal with index finger.

What to do next — not a dead end

Suggestions use body region, goal, motion type, and allowed phases — not your medical record. After surgery or a flare, follow your clinician first.

Estimated time

Equipment

None required — table or bodyweight only.
